How To Apply

The following steps explain how to apply for the National School Lunch Program and School Breakfast Program.  You may also refer to our list of things to consider before applying [ pdf 31.6 kB ] for more information.

Charter schools must have the following to be approved as a sponsor.  

  1. SAM/DUNS Registration [ pdf 333.4 kB ]
  2. Online Substitute Form W9 — Vendor Registration [ word 61.5 kB ]
  3. MyFloridaMarketPlace [ word 62.8 kB ]
  4. Copy of signed charter school agreement between the charter school and the school district
  5. School must be registered with the Florida Department of Education.
  6. Department of Health Food Operating Permit  — Please contact your local health department.

Private nonprofit schools must have the following to be approved as a sponsor.  

  1. SAM/DUNS Registration [ pdf 333.4 kB ]
  2. Online Substitute Form W9 — Vendor Registration [ word 61.5 kB ]
  3. MyFloridaMarketPlace [ word 62.8 kB ]
  4. IRS Letter of Tax Exemption — 501(c)(3) 
  5. School must be registered with the Florida Department of Education.
  6. Department of Health Food Operating Permit — Please contact your local health department.

Residential child care institutions must have the following to be approved as a sponsor.

The National School Lunch Program Sponsor Training is a series of online training courses designed specifically for the Administrative/Food Service Director and Operational/Cafeteria Management staff who will be responsible for implementing the National School Lunch Program. Both administrative and operational staff members are strongly encouraged to complete the training courses. This training is required for schools and residential child care institutions that are interested in participating in the National School Lunch Program, School Breakfast Program and Afterschool Snack programs.

Prospective sponsors can register for the online training courses at the National School Lunch Program Training Site.

  1. Submit the required documents for review, which are located at https://fans.fdacs.gov
  2. Once your required documents have been submitted and approved, an onsite preoperational visit will be set up with your Program Development Specialist.
  3. Complete the online child nutrition program module.
  4. Once your online application has been reviewed and approved, you will receive an approval confirmation from your Program Development Specialist.
  5. After receiving the approval confirmation, you may begin claiming reimbursable meals served to students.
