If your license has been lost, stolen or destroyed, you can obtain a replacement license by contacting the Florida Department of Agriculture and Consumer Services.

Replacement Fee

The fee for a replacement license is $15.

How to Request a Replacement License

• You may visit one of our regional offices (notarization services are available);
• You may visit an authorized tax collector location (please contact your local tax collector for information regarding availability of services); or 
• You may mail your request to this address:

Please include the following items:

• A written statement explaining that your license has been lost, stolen or destroyed (this statement must be notarized). We ask that you provide your full name, date of birth, address, and telephone number. If you have your concealed weapon license number, please provide it as well. 
• The $15 replacement fee (paid by check or money order made payable to the Division of Licensing).
