The Florida Department of Agriculture and Consumer Services offers Florida horse owners two options to extend the expiration date of their animal's "health certificate," more properly known as an Official Certificate of Veterinary Inspection.

Horse owners traveling out of state may still apply for an Extended Equine Certificate of Veterinary Inspection (EECVI) through their private veterinarian in conjunction with Global Vet Link. For more information, visit the Global Vet Link website.

Additional EECVI vendors may eventually become available through an approval process by the United States Animal Health Association.

Negative EIA Test Verification Card

FDACS offers Florida horse owners a convenient laminated card, the Negative EIA Verification Card, as proof of the required negative equine infectious anemia (EIA) test. This card is convenient for trail riding and equine movement within the state and can easily fit in a wallet or pocket.

The Negative EIA Test Verification Card  is $5 per card.

Photo Requirements 

The Negative EIA Test Verification Card requires digital photos that show the horse’s entire left side, entire right side and front view of the entire head. Each view should have approximately 10 percent background bordering all four sides of the image.

Submitting Your Application

Submit your application by mail to the address on the application form.

Include a check or money order with the appropriate fee, or you may pay by credit card using our online payment system.

Pay Negative EIA Test Verification Card Fees Online.

Please mail photos on a CD or thumb drive or email them to EquinePrograms@FDACS.gov.

The photos and application may also be sent through the online payment system. If submitting online, please submit all documents as a single file.
