Florida Official Certificates of Veterinary Inspection (OCVIs), also known as health certificates, may be completed only by currently licensed Florida veterinarians accredited under the National Veterinary Accreditation Program.
Currently licensed and accredited veterinarians may order health certificates from the Florida Department of Agriculture and Consumer Services using the Health Certificate Order Form [ Adobe PDF Document ].
Official Certificates of Veterinary Inspection are available electronically or as paper documents. Please review options available for
