Replacement Fees

• Agency License: $10
• Individual License: $15

How to Request a Replacement License

You may visit one of our regional offices to obtain a replacement individual license or submit your request for a replacement agency/school license. Agency/school licenses will be printed and mailed.

Please include the following items:

• A written statement explaining that your license has been lost, stolen or destroyed. We ask that you provide your full name, date of birth, address, and telephone number. If you have your license number, please provide it as well. 
• Your replacement license fee (paid by check or money order made payable to the Division of Licensing).
