Provides helpful information about setting up an account and the online Employee Action Report feature.
The following information is provided to help you use Employee Action Report (EAR) system.
Log in to Employee Action Report System
- Select the Employee Action Report link.
- Enter your account number and password.
- Select Log In.
Change Your Password
- Log in to your account to access the Employee Action Report Main Menu.
- From the Main Menu, select the Account Information link in the left navigation panel.
- Select Change next to your password in the Account Information box at the bottom of the page.
- Enter your new password in the New Password field and the Verify Password field.
- Select Update.
Forgot Your Password?
- Select the Employee Action Report link.
- Select the Forgot Your Password link.
- Enter your account number and Social Security number.
- Select Log In.
- Enter your new password in the New Password field and the Verify Password field.
- Select Update.
Employee Action Report Main Menu
- After you have successfully logged in, the Employee Action Report Main Menu will display.
- Select the agency license you will use for the current session.
- Select the desired employee reporting action: Hire, Terminate, or Change Address.
- Select Continue.
- Use the links in the left navigation panel to see your Account Information, perform another Employee reporting action, and view your agency's employee list.
What Is the Format of the License Number?
- The format of the license number is the license prefix and a seven-digit number.
- If the license prefix is one letter (C, D, E, G, M), type the letter followed by a space. For example: The format of a "D" license is "D 1234567."
- If the license prefix is two letters (CC, EE, MA, MB, MR), type both letters with NO space. For example: The format of an "CC" license is "CC1234567."
Hire an Employee
- Enter the employee's license number and Social Security number.
- Verify that the information is correct.
- Enter the hire date in MM/DD/YYYY format.
- Select Confirm.
- If no errors are encountered, a message is displayed to indicate that your transaction was successful.
- You may perform another employee reporting action by using the Employee Action Report link in the left navigation panel.
Terminate an Employee
- Enter the employee's license number and Social Security number to be terminated.
- Verify that the information is correct.
- Enter the termination date in MM/DD/YYYY format.
- Enter resignation indication (Y=Yes or N=No); default is N=No.
- If the termination was due to a 493 violation, check the appropriate cause for dismissal.
- Select Confirm.
- If no errors are encountered, a message is displayed to indicate that your transaction was successful.
- You may perform another employee reporting action by using the Employee Action Report link in the left navigation panel.
Change an Employee's Address
- Enter the employee's license number and Social Security number.
- Verify that the information is correct.
- Enter the zip code for the new residence and/or mailing address.
- Select Zip Code.
- Enter the new address information.
- Select the desired city from the dropdown list provided.
- Select Confirm.
- If no errors are encountered, a message is displayed to indicate the address change was successful.
- You may perform another employee reporting action by using the Employee Action Report link in the left navigation panel.
View Agency's Employee List
- From the left navigation panel, select Employee List.
- If the agency has more than 50 employees, select Next List.
- You may perform another employee reporting action by using the Employee Action Report link in the left navigation panel.
What Should an Agency Do When a New Employee Has Not Yet Been Assigned a License Number?
- An agency can update an employee's record only if the employee has a valid license number or an application on file.
- If the person you are hiring has recently submitted an application to the Division of Licensing, wait a few weeks before you report the hire using the Employee Action Report system.
- Use the Individual Search tool to locate the license number assigned to the recently hired employee. If a license number has not been assigned by the Division of Licensing within 14 business days, complete an Employee Action Report form, FDACS-16006 [ Adobe PDF Document ], and mail it to the Division of Licensing for further research to be performed.
